Trust failed to provide adequate care and treatment to patient during last weeks of her life

An investigation has found a number of failures by the Southern Health and Social Care Trust in relation to a patient’s care and treatment, and that appropriate and timely intervention may have alleviated her discomfort and distress in the last weeks of her life.
