Failings by Trust caused patient's family upset and frustration

A woman complained about the treatment her father received in Altnagelvin Hospital, including that he should not have had a catheter inserted while he was dying.  We upheld parts of the complaint.

Report Recommendations

We asked the Trust to apologise to the patient's family for the injustice caused.

To prevent similar failings from happening again, we asked the Trust to share the report's findings with:

  • Nursing staff - to ensure appropriate communication with family members of patients who have passed away
  • Clinical staff - to highlight the findings on the timing of catheter insertion
  • Complaints Officers - to ensure appropriate communication between the Complaints Team and front-line staff undertaking informal complaint resolution.
