Investigation of a complaint against the Southern Health and Social Care Trust

Our investigation found that the communication between clinicians caring for a patient in Craigavon Area Hospital was inadequate, and that a series of other failures contributed towards his untimely death.

Report recommendations

We asked the Trust to apologise to the complainant for the failures in his father's care.  

We also asked it to say sorry for not doing a thorough investigation into what happened, and for failing to follow relevant guidance when reporting a Serious Adverse Incident.
