Investigation of a complaint against the Western Trust

A woman complained about the care her late husband received in hospital during his treatment for cancer.

Report recommendations

We upheld parts of the complaint.  We found that the Trust failed to provide the patient and his wife with the information they needed to help them make informed decisions about his palliative care.  We also found the illegibility of some of the Trust's written records to be a failure in the patient’s care and treatment. 

Our recommendations included for the Trust to:

  • Discuss the findings of our report with all clinicians involved in the patient’s care, and to;
  • Remind staff of the importance of maintaining legible records
