A Health Trust’s failure to carry out tests on a man with an undiagnosed heart condition left his family with unanswered questions following his death.

We recommended that the Trust carry out a Serious Adverse Incident Report into the death of the patient to help it identify learning.
We also asked it to discuss the findings of our report with all clinicians involved in the patient’s care and to provide training to relevant staff about the importance of reaching a diagnosis prior to discharging high risk patients.
