Trust to apologise for failures in care of diabetic patient

The Northern Trust failed to keep proper records of a patient's food intake, and didn't provide him with insulin after he experienced an episode of hypoglycaemia.

Report recommendations

For service improvement and to prevent similar failings in the future, we asked the Trust to:

  • Remind staff of the importance of maintaining accurate and complete food intake charts when caring for vulnerable diabetic patients with variable oral intake.
  • Carry out a random sampling audit of vulnerable, diabetic patients on the Ward in question.
