Trust did not properly diagnose patient before discharging her from hospital

We upheld a complaint from a woman who said that her late mother should not have been discharged from Antrim Area Hospital.

Report recommendations

We asked the Trust to apologise to the patient's daughter for the distress caused.

We also asked it to ensure relevant medical clinicians faced with similar cases are reminded of the importance of considering delirium as a diagnosis.
