The care of a patient by the Southern Trust’s Home Treatment Crisis Response Service

A complainant raised concerns about how the Southern Trust cared for her vulnerable sister.  

Our investigation found a number of failings by the Trust.  

Report recommendations

The failings identified in this report are of significant concern and we would expect the Trust and its clinicians to learn from what happened.

We asked the Trust to apologise to the complainant, and to:

  • Highlight to staff the importance of incorporating the views of patients and their loved ones when developing care plans
  • Take a random sample of similar cases over the last six months to check if there is evidence of such views being recorded within the care plans.
