‘Significant failings’ in care of service user with special needs
A man said his autistic son’s health deteriorated because the Western Trust did not properly address his needs. Our investigation found that the Trust failed to provide a reasonable level of care and support to both the boy and his family over a 21-year period.

Report recommendations
We asked the Trust to apologise to the man for the multiple failings we identified.
We also recommended it review a number of its processes and incorporate them into an action plan which it should submit to us within 6 months. These included the process of managing a client's transition from Children's to Adult Services, and how it carries out assessments in its Adult Learning Disability Service.
The Trust accepted our findings and recommendations.
