Trust’s failures led to patient’s untimely death

The Northern Trust failed to act on a woman’s concerns about her mother’s health.  This, as well as its failure to properly assess her spinal injury and accurately read the results of an MRI scan, led to her untimely death.

Report recommendations

The issues identified were of significant concern, so we made a number of recommendations.

We firstly asked the Trust to apologise to the complainant for the failures in the care of her mother.

We also asked it to carry out a Serious Adverse Incident (SAI) investigation to cover the parts which were not included within the scope of the original SAI.

We asked the Trust to look at how its cardiology unit gives appropriate specialist pain management advice, documents the detail of its clinical findings, and records discussions with other professionals about patients.

Finally we recommended that the relevant staff involved in the care of the patient were given the chance to reflect on the report's findings.
